Do not use this form for the U11, U12 or U13 Tournaments. They have their own forms.
A check for $600 must be mailed to MAPLE soccer. The check will be returned if the team does not qualify for the tournament. The office address is 85 Cottage St, Natick, MA 01760
A separate check & declaration form must be submitted for each team. The application deadline is May 15th.
Please note: The declaration form must have your current age group, not the one you are playing into next year.
